How to report an issue

Identify the exact URL, disputed statement, reason for the challenge and the best primary source available. Do not submit account credentials or personal financial records.

How we review

An editor compares the original source, new evidence and the claim’s context. Providers receive no special correction standard, but credible primary evidence is evaluated promptly.

Material corrections

Changes affecting a score, legal entity, license status, custody conclusion, fee calculation, incident record or central verdict receive a visible note and updated verification date.

Minor edits

Spelling, formatting and clarity edits that do not alter meaning may be corrected without a separate log entry.

Appeal

A challenged decision can be reviewed again when new evidence is supplied. Repetition without new evidence does not require a new outcome.

Challenge the record.

Send the exact page, disputed claim and best available source. Material corrections will be logged visibly.
